Introduction to UFS Medical

First, do no harm.

While the oath that doctors have taken over centuries has evolved, the core tenant has never changed: A doctor’s duty is to protect and preserve life. Doctors working on ships and stations carry on a tradition that goes back, nearly to Earth’s first century, while those working in the colonies honor a medical legacy that goes back nearly to Earth’s prehistory. Far from outside perception of doctors standing around empty medical bays awaiting the next emergency, stepping into the teal uniform of UF Starfleet Medical means stepping not only into the shoes of the great doctors and nurses of the past, but also standing as a direct barrier between the crew of their ship and the often invisible dangers encountered in space travel and exploration. Far from being a band-aid dispenser, a Starfleet Medical officer’s duty is to attend to the complete health of all the people present on their ship, station, or in their colony. It is the duty of UF Starfleet Medical to stand between the population under their care and disaster.

Within the medbay, doctors and nurses follow their own chain of command, which can, at times, put them at odds with the expectations of those outside of the medical bay. The Chief Medical Officer also has the ability to declare anyone unfit for duty when certain criteria are met, a responsibility not to be taken lightly, and one which means that the CMO, far from being a distant figure hidden away in the medbay, must be in close communications with the command staff. When faced with a problem, there are two clear methods to be used: one for the individual patient and another for mass casualty incidents.

The first tool in a medical officer’s bag is diagnosis following simple steps;

  • Assess patient history
  • Run base biobed scans and receive history of symptoms
  • Perform more targeted tests, begin acting on results.
  • Reach a diagnosis and treatment plan.

With these steps complete, the attending has the ability to consult with the other doctors, provide discharge information, enforce monitoring, and document the steps taken.

The second tool, and arguably the one that we hope needs to be used the least, is that of Triage. Starfleet follows an adjusted standard for Triage practice, following the now-classic Green, Black, Red, Yellow tag system.

  • Green: The so-called ‘Walking Wounded’, injured, but not in any significant danger if left untreated for a time.
  • Black: The dead or those that cannot be saved from their injuries.
  • Red: Those that will die within a very short period of time if left untreated.
  • Yellow: Those whose injuries are significant, but can likely wait a few hours for treatment if necessary without significant risk of death.

With these two tools in hand, any medical officer can handle most anything that comes through their door. Consider the following:

A patient comes in complaining of pain in their eyes, presenting with reddened sclera and swollen eyelids. While you set the biobed to get a current scan, you pull up their medical history on your PADD, checking for any previous incidents that may be related, and asking the patient questions relevant to their complaint. As you observe them, you notice that they’ve been rubbing at their eyes, meaning that the red sclera and swollen eyelids may be unrelated to the complaint. Initial scans show nothing amiss, but upon closer look at the eyes you discover a foreign object inside the vitreous humor that is likely the source of the problem. A simple surgery removes it, but you run a sample through analysis to ensure that the little crystalline subject isn’t a problem that will repeat. When it gives some odd results, you carry the data over to the Sciences division and ask for comparisons. The results from there may bring about final diagnosis, or add more questions and complications. Each step becomes a puzzle for the medical team to solve.

An introduction to UFS Medical, an OOC Perspective

The number-one fear many have about joining medical is that they are going to mess it up. They’ll say the wrong thing, do the wrong thing, and ruin it all. A slippery down-hill slope. But the joy of Medical is that you are never, ever alone.

Medical is all about communications and creativity. Communication because it is not up to you whether a character lives or dies (within reason, of course!), but their player. So the moment the above person walked into the medical bay, I would have IM’ed them, asking if they had a specific goal. Some have a character change, some have something their character is still working out from previous missions, others just decide they want to give medical something to do (and bless them for that). This does NOT mean that they get to decide their full course of treatment. If a member of the crew has been blinded and wants ocular implants, but also wants to be perfectly healthy within the next hour, that’s not possible. Ultimately it is the doctor who decides the full extent of the consequences of an injury or treatment, in consultation with the players involved.

As to the technical aspects, well, there’s a combination of bluffing, knowledge, and behind-the-scenes googling that can solve that problem for you. For the above, knowing I’d be doing an eye-exam, I’d perhaps look up a diagram of the eye so I’d know the names for the parts, then use more technical descriptions for the problem. In my own roleplay, I’m frequently consulting both Memory Alpha, the UFS websites, and various google combinations of Star Trek and real world problems to find previous treatments. Have a blind patient? Googling “Blindness Star Trek” Will bring up numerous entries on episodes that featured just that problem, or characters with that problem, and the solutions employed. You don’t need to memorize every tool, you just need to learn to utilize the tools at your disposal to make it seem like you did.

Having said that, I feel the urge to remind you, dear reader, that we are not islands unto ourselves. I am always sending questions to Science division, or to my CO. I frequently receive questions from fellow medical officers, and if I don’t have the answer, I’ll ask others myself. Doctors in the real world today almost never work alone, they are always consulting with their nurses, other doctors, databases, and anyone who can help them figure out a solution for their patient. When running the medical bay, I always have a separate chat open with my fellow doctors, ensuring we don’t step on each other’s toes or feel left-out. Remember, in UFS you are part of a team, and it is my job (and the job of all those CMOs and Medical Branch Commanders who follow me) to ensure you are supported. We are UFS, strong together, not apart.

UF Starfleet Medical Position Info

All Starfleet stations, ships, and colonies will have at least one doctor aboard whose duty it will be to ensure that the crew is kept healthy, to treat all illnesses and wounds, and to assess the physical and mental capability of all officers and personnel under their care.

In contrast to other areas of the ship, the medbay is one virtually without formal titles. With the exception of the Chief Medical Officer and Vice Chief Medical Officer, almost everyone is referred to by the status of the medical training: as doctor or nurse. Despite Dr. Crusher’s rank of Commander, and occasional position as Officer of the Watch, she was rarely referred to by her starfleet rank, almost always addressed as Doctor first. Chain of command within the medical bay follows this structure:

Chief Medical Officer (CMO)-> Vice Chief Medical Officer (VCMO)-> Doctor>Nurse -> Emergency Technician.

Office Staff

Branch Commander

The Branch Commanders of UFS are leaders in the immersive experience offered by United Federation Starfleet. They do this by offering their knowledge and passion of the aspects of their branch to other members in the form of Role Play, Activities and Education.

Role Play can vary from something as simple as in character treatment discussion, to reports on new species DNA to having to coordinate a trauma. Fun Activities is such a huge catch all it defies solid definition but ideas can range from things like keeping up to date and sharing interesting news stories and development of real life technology like the Medical Tricorder, hosting some get togethers to chat on particular things seen in an episode of Star Trek, the list is endless. They also work with UF Starfleet Academy to assist in the development and maintenance of the content in the colleges.

The Branch Commanders are also the primary representative of the branch to UFS Command, advising them on matters of the branch and the members in it by maintaining communication with both.

Vice Branch Commander

The Vice Branch Commander is responsible for assisting the Branch Commander with the day to day organization and running of the branch. Often a supportive role the Vice Branch Commander will mimic the Branch Commander in their absence and work alongside them to achieve goals and objectives.

Positions in UF Starfleet Medical

Chief/Vice Chief Medical Officer

Responsible for the physical well being of the crew, the Chief Medical Officer serves as chief physician, a medical researcher, and more as well as heading up the Medical department. S/he, along with the XO, is one of the only officers who can remove a CO from duty.

Surgical Specialist

The Surgical Specialist is a Medical officer who is highly skilled as a surgeon. This individual is involved in all surgical consults, and performs surgery on crew members when necessary.

Trauma Specialist

The Trauma Specialist is held by a Medical officer who is knowledgeable in all forms of medical treatment as it relates to dealing with traumatic injuries, or injuries acquired during combat or hostile situations. This crew member is the first to attend to such victims, and is one of the few who can commandeer sections of the ship to turn it into a triage.

Pathology Specialist

Pathology Specialists have two major functions. First, they are responsible for performing autopsies on deceased crew members, plus preparing the bodies for funeral/memorial services - either on board the ship, or with their families. Second, they are responsible for investigating all airborne threats, including new strains of viruses introduced with the intent of killing many.

Pediatric Specialist

The Pediatric Specialists have one of the best, and probably most rewarding jobs as part of the Medical department. They help to oversee the delivery of babies, and then oversee their treatment and care, for as long as they are on the ship or until they reach adulthood.

Medical Officer

Responsible for assisting the Chief Medical Officer with examinations, administration, etc. in order to ensure the physical well being of the crew.

Medical Technician

Medical Technicians spend most of their time in the medical labs, running various experiments on samples, ranging from tissue to organs. They are often charged with uncovering the source behind an illness.

Medical Researcher

It's a large galaxy out there, and the number of illnesses from across the universe can be astronomical! The duty of the Medical Researcher is to keep on top of all types of illnesses, from among all species. They look for similarities between them, and current illnesses that may be plaguing the crew.

Head Nurse

A Head Nurse has usually served as a Nurse on board a starship for a number of years, or has had extensive experience within the medical field, or in managing other nurses.

Nurse

Trained in basic medical care, these NCOs assist the medical staff in operations, and are able to treat minor injuries and such on their own. They also maintain the medical ward and deal with the trivial tasks to allow the medical personnel to focus on the real problems.

Billet Requirements

Each role in the branch requires a certain amount of understanding of the area someone is assigned to. These roles are what we call “billets” and the following is the suggestions for each Billet.

The medical department is responsible for the physical health of the crew, from running annual physicals to combatting a strange plague that is afflicting the crew.
